Summary:
Responsibilities include resource assessment to identify client needs and establish connections/rapport within the community to provide myriad resources and connection to services.
Case managers will be responsible for service planning and service plan implementation & coordination, as well as monitoring and follow-up on these services.
Case manager is expected to be active in aftercare planning, providing assistance with any legal issues, initiating FMLA/Short Term Disability Claims and third-party requests.

Responsibilities:
General Case Management functions supporting client needs.
Collaborating with Clinical Supervisor on important collateral needs.
Attend daily huddle meeting to coordinate daily case management needs Coordinate/facilitate Third Party records requests.
Coordinate ancillary services Participate in weekly Treatment Team Meetings.
Assist clients with securing resources.
Discharge & Aftercare planning.
Maintain communication with families, patients, and staff.
Initiate and/or follow up on existing FMLA and Short Term Disability Claims Assist with Unemployment if applicable Assist with Cobra enrollment if applicable Core Competencies:

Full-time Preferred education:
Associates Degree or college coursework related to this position, Bachelor's Degree Preferred.
Preferred
Experience:
2-4 years' experience working in case management, preferably with a mental health population Proper Knowledge and Skills:
Proper English and grammar, usage and spelling Software application related to the field General offices principles and practices Basic computer knowledge including email etiquette Professional phone and customer service demeanor Microsoft Word, SharePoint, Excel, and Outlook Required licenses or certifications:
Current CPR Certification Narcan Certified.
Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
